Rose Kakoza, M.D., MPH, Named Senior Clinical Network Director, Population Health
Rose Kakoza, M.D., MPH, has been appointed senior clinical network director, Population Health. In this role, she is responsible for the strategic development and growth of the Delaware Medicaid Partners Accountable Care Organization, ChristianaCare’s Medicaid ACO.
This includes the design and scale of effective models of care delivery and the development of innovative population health management programs and services focused on the unique needs of Medicaid patients to drive optimal health while controlling costs.
She also will provide strategic clinical direction and oversight over CareVio case management services that support all value-based programs. This includes driving payer relationships to ensure efficient, coordinated deployment of case management and disease management services across multiple lines of business.
Dr. Kakoza brings extensive experience to this role, previously serving as medical director for ChristianaCare’s Medicaid value-based care contracts. Prior to joining the Population Health Group, she served as clinical director of Community Health & Equity for the Primary Care and Community Medicine Service Line.
She formerly served at Brigham & Women’s Hospital and Partners HealthCare and was on faculty at Harvard Medical School.
Dr. Kakoza has published and presented on primary care, community health and population health.
She reports to Christine Donohue-Henry, M.D., MBA, president and CEO of eBrightHealth ACO.
