“We really see this ACO model as an opportunity to optimize our care delivery to the Medicaid population and really work to eliminate those healthcare disparities that we see in our community,” said Kakoza.

“Key to the success of Delaware Medicaid Quality Partners ACO will be building on our strong clinical partnerships with providers in primary care, pediatrics and behavioral and women’s health, as well as continuing to work closely with our community-based partners to provide health care access and support in the communities where patients live,” said Rose Kakoza, M.D., MPH, medical director for Delaware Medicaid Quality Partners ACO in a statement. “Among areas we will especially focus on are health equity, care standardization and addressing social determinants of health.”

The Delaware Medicaid Quality Partners ACO will seek agreements with the state’s two Medicaid managed care organizations, AmeriHealth Caritas Delaware and Highmark Health Options. The contracts are expected to begin July 1, 2021 and run through Dec. 31, 2024.

“Virtual care is an opportunity for us to meet people where they are and provide excellent and equitable care in trusted community spaces,” said Rose Kakoza, clinical director of Community Health and Equity for Primary Care and Community Medicine at ChristianaCare. “Virtual care also allows us to continue to care for COVID-19 positive patients in their homes and connect all patients with primary care providers.”
