Carla Aponte Johnson Named Director of Community Health and Social Integration
Carla Aponte Johnson has been appointed as director of Community Health and Social Integration. In this role she is responsible for the oversight and growth of all social care programs that are integrated within the clinical space. In addition, she leads the work to partner across the organization to provide social care integration within new community-based care models. This work is central to the mission of ChristianaCare and is responsible for ensuring that we are providing community members with the ability to identify and meet their social needs so that people can live their best and healthiest life.
Aponte Johnson joined ChristianaCare in 2011. In that time she has educated pregnant women and adults on diabetes as a community educator; led the Health Ambassador and Health Guide programs while implementing the Affordable Care Act and Health Insurance Marketplace; and worked as a senior program manager to develop the inaugural Community Health Worker team. As a manager, she has overseen the growth of that program with more than 40 full-time equivalents in multiple clinical sites.
She has been a key contributor to the development, implementation and success of many other Community Health programs like the Produce RX program, Medical-Legal Partnership and Boot Camp for New Moms and Dads. During the spring of 2021, Aponte Johnson led the teams at ChristianaCare’s COVID vaccine community events, which vaccinated more than 5,000 community members.
Aponte Johnson has a bachelor’s degree in photojournalism from Pace University and a master’s degree in Health Care Administration from Wilmington University. She reports to Erin Booker, LPC, vice president of Behavioral Health and Social Care Integration.
