Q. What if the unsafe practice or condition cannot be fixed immediately?
A. Develop and implement a unit/ department action plan to correct within 30-60 days.
For example:
• Any recurring unsafe conditions.
• Unit/department rounding or tracer findings.
• Incomplete medical record documentation.
• Practice not matching/consistent with policy or procedure. 
Q. What is FIND IT ... FIX IT?
A. FIND IT ... FIX IT is a new strategy to promote staff engagement to take ownership of safety and regulatory risks and to take action immediately.
Q. What is meant by “safety and regulatory risks”?
A. Safety and regulatory risks are any conditions, situations or actions that can potentially result in harm or injury to patients, visitors or staff.
Q. What actions can I take?
A. If you see or become aware of any condition that can potentially result in harm or injury to patients, visitors or staff, immediately take action to correct the condition. Actions may include:
• Correcting any unsafe condition immediately, if it’s within your scope.
• Submitting an R2L report.

• Removing and reporting expired equipment to Clinical Engineering.
Q. What are examples of things I can correct immediately?
A. Examples of conditions that you can correct immediately are:
• Securing any freestanding oxygen cylinders that you see by placing in a designated rack, cart or other approved holder.
• Removing any unsecured oxygen cylinders lying on patient stretchers or beds and placing them in designated holders.
• Moving items that block the emergency exits or the path to the emergency exits.
• Removing ripped/torn furniture/ stretchers from service and submitting a work request.
